Dear Parents:

Thank you for providing us with all the information and doctor’s orders to care for your child this year.

Each school year we are required to have new and current physician’s orders in place.  For this school year, please have the physician fill out the appropriate forms, medication authorization and /or specialized health care procedure forms. The medication forms need to be completed for all over the counter and prescribed medication (including topical ointments) that your child will be needing at school. Please do not send your student to school with ANY prescribed or over-the-counter medications without proper medical authorization approved by the school's health office.
