MIDDLE SCHOOL (6-8) DEVICE REQUEST FORM

DOES YOUR CHILD HAVE AN AVAILABLE DEVICE AT HOME?

  • If you currently have a digital device, such as a desktop computer, laptop, Chromebook, IPad, Galaxy tablets and other Android-based devices that can connect to Chrome in your home available for your child’s use, you DO NOT NEED to complete the form.

  • If you need a device for your child, PLEASE COMPLETE the online form as soon as possible so that we can ensure devices are ready to pick up consistent with the schedule outlined below.

  • If your child is serviced by an individual education plan (IEP) and has a designated assistive technology device per their IEP, PLEASE COMPLETE the online form, as well, so we can be sure the device is ready for pick-up.

Last names starts with:

  • A ... 8:00-9:00
  • B-C ... 9:00-10:00
  • D-F ... 10:00-11:00
  • G-K ... 11:00-12:00
  • L-M ... 12:00-1:00
  • N-R ... 1:00-2:00
  • S-T ... 2:00-3:00
  • U-Z ... 3:00-3:30 (plus any that missed your time earlier in the day)

We will have a drive-thru system with four pick up stations AND one walk-up station. We anticipate the drive-thru system being the quickest, most efficient way to pick up your Chromebook. 

The TMS Bus Loop is our designated drive-thru pick up area. Please enter the bus loop from southbound on Meadows Parkway and do NOT use the left hand turn lane from the northbound direction. 

Our parking lot will be closed and shut down during this time. 

The walk up station is also in the bus loop area. 

Social Distancing:

Drive-thru: You will be directed to a station in the bus loop. From inside your car, indicate to staff the name of the student(s) you are picking up for. Remain in your car until the device has been placed on the pick-up table and the staff member has returned to their area and signals you with a thumbs up. At that time, you may retrieve that device from the table and exit.

Walk-up: please line up maintaining 6 feet of distance from the person in front of you. Follow the posted directions.

We plan to check in devices as soon as the “stay home” order has lifted and we return to school.
