To request a non-student Public Record, please complete a Records Request Form located below.
To the best of our ability, the records provided will be in an electronic format accessible solely through the completion of the form below. If printed copies are requested or the only format available, a $.10 cent per page charge will be applied and collected prior to the release of documents.
DO NOT USE THE FORM TO REQUEST IMMUNIZATION RECORDS OR OTHER STUDENT RECORDS. Please click on the "Student Records/Transcripts" link in the left-hand menu for directions on how to obtain student records.

Public Record Defined:
The public may inspect or obtain a copy of identifiable public records. Writings held by state or local government are public records. A writing includes all forms of recorded information that currently exists or that may exist in the future.
