Benefit plans are available until the age of 65 or when you become eligible for Medicare, whichever comes first.  Please note that you become eligible for Medicare on the 1st of the month in which you turn 65.

• CVT Change Form For removing family members
• Kaiser Enrollment Form Complete when switching from a PPO plan to Kaiser
• Delta Dental Form For removing family members
• VSP Vision Form For removing family members

Thinking of retiring?  If you meet the following requirements, you may be eligible for the "Bridge Benefit", which helps reduce the cost of your health insurance.  

1. Are between the ages of 55 and 65
2. Worked for TVUSD for the equivalent of 12 full-time years
3. Have been enrolled in medical insurance through TVUSD for the previous 10 years
4. Must provide 90 days notice of your retirement.

Retirement Forms - Please return them to your benefits technician at least 90 days prior to your retirement date.
