Student work permits are available at the school sites during the student calendar school year.
Student Welfare & Success can issue them ONLY during breaks in Winter, Spring and Summer
M - F from 8:00am-12:00pm or 2:00pm-4:00pm.

Work Permit Instructions
A STUDENT MUST ALREADY BE HIRED BEFORE A WORK PERMIT CAN BE ISSUED
ONCE HIRED:

The student, employer and parent must complete the STATEMENT OF INTENT TO EMPLOY A MINOR AND REQUEST FOR A WORK PERMIT–CERTIFICATE OF AGE CDE Form B1-1 (Rev. 02-14). There are a few different ways a student can get this form.

From the new hiring employer

From the following link: CA department of Education website

From a school site representative (during Winter/Spring/Summer break, the district office/Student Welfare & Success representative can provide it.)

This must be completed before a work permit can be issued.

Be sure to check each line of the form.

The employer must complete & sign their section:“To be filled in and signed by the employer”

Parent/legal guardian signed their section.
-
The school or district representative will complete the bottom section – maximum hours allowed to work and verify the student’s DOB.

The signed original goes to the school/district representative, who will prepare & issue the actual Work Permit for the student/minor’s signature.

The school/district rep. will make copies and distribute them accordingly.
