Daily Required Self-Assessment Health Screening for On-Campus Students and All TVUSD Staff
ALL TVUSD on-campus, in-person employees, students, and visitors will be required to complete a daily self-screening health assessment before entering a school campus, district office department//MOT, or any other TVUSD location. The self-assessment measures and contact tracing are in compliance with CDC and CDPH guidance to help mitigate the impacts of potential exposures and limit the number of individuals that are required to quarantine, when possible.
As of January 1, 2021, AB 685 requires notification to employees of exposure and notification to the local health department of an outbreak in the workplace. To streamline compliance with this requirement, TVUSD is contracting with Qualtrics to provide the required daily self-assessment communication and any follow-up contact tracing notifications. All contact information is confidential and maintained by TVUSD and will be used for contact tracing only.
The daily self-screening assessment will be delivered via email to all on-campus students and all employees via their TVUSD email accounts every weekday morning. It will come from the following email address: TVUSDDAilySelfScreeningAssessment@qualtrics-survey.com
If you forget to complete your daily health self-screening before reporting to school or work, all school sites and district offices will have the individual QR codes posted flyers at reception/front office doors, all ingress & egress areas (as per site safety plan) to allow for additional access to the TVUSD self-screening assessment via a mobile device. Itinerant or roving employees will need to scan the QR code on the posted flyer at each site/department before entering. However, after you complete the initial daily health screening BEFORE reporting to your first site, the system will bypass the health screen portion and check you in at that site or department.
