Application for Temporary Disability Academic Program

We have added a new convenient way to apply for our Temporary Disability Academic Program (TDAP) at TVUSD.

Please note that before you can submit this application, you must obtain a physician's note and counselor's recommendation. 

These must be uploaded to your application.   

To obtain the Counselor's Recommendation form, please reach out to your child’s school counselor and request it. 

Your application will be incomplete without these documents!
