  • Welcome to the Crowne Hill Health Office. Our office is staffed with an itinerant District Nurse, Natalie Whitworth RN, and a Health Clerk on site daily, Jeri Ognjanac. Please feel free to contact either of us with any questions or concerns you may have regarding the health of your child.   • Dear Parents:

    Thank you for providing us with all the information and doctor’s orders to care for your child this year.

    Each school year we are required to have new and current physician’s orders in place.  For this school year, please have the physician fill out the appropriate forms, medication authorization and /or specialized health care procedure forms. The medication forms need to be completed for all over the counter and prescribed medication (including topical ointments) that your child will be needing at school. Please do not send your student to school with ANY prescribed or over-the-counter medications without proper medical authorization approved by the school's health office.

Student Wellness

  • "Healthy students learn better." Although it is important for students to be at school every day, the health staff would like to review the general guidelines for students coming to school. Please take a few moments to read the following protocols.

    Students should not be sent to school with the symptoms listed below:

    • Fever (100 or above)
    • Vomiting
    • Diarrhea
    • Nausea with or without vomiting
    • Pain that is not controlled
    • Flu symptoms
    • Infectious Conjunctivitis (pink eye). This requires a physician's visit.

    Students may return to school:

    • Fever free for 24 hours without medication
    • No vomiting for the last 24 hours
    • Infectious Conjunctivitis (pink eye) when antibiotic treatment has been administered for at least 24 hours and symptoms have subsided.

    It is the District's goal to ensure the safety of students with injuries or other medical conditions that require splints, casts, crutches, or other orthopedic devices (i.e. wheelchairs, scooters, etc.).
    These conditions require medical care and treatment. To ensure the safety of all students, TVUSD requires a medical provider release for the student to return to school. It is the parent's responsibility to provide the school with medical documentation that allows the student to return to school. Documention should state the reason for the use of the assistive device, the estimated duration of use, and any activity limitations.
    Medical documentation should be submitted to the health office. Please see the Physician's Recommendation form under the Health Forms section.  If there are any concerns or questions please contact the Credentialed School Nurse assigned to the school site.   

    Due to unexpected illnesses and emergencies, current work, home, and cell phone numbers are important to our school. Please take the time to update any changes in your personal phone numbers and emergency contact numbers.
