• TTES Medication and Doctor Orders 

    Hello Trailblazers Parent or Guardian,

    We are so excited to have students back in school! Remember that If your child needs to receive medication while at school, you must bring it to the school, along with a physician’s medication order signed and dated for the remainder of the school year. You can drop off the medication and physician’s order to Tony Tobin Elementary School starting today, August 9  from 9:00am-3:00pm. If your physician would like your child to carry either an asthma inhaler or emergency medication (auto-injectable-epinephrine, i.e. Epipen), Part III of the medication administration form must be completed by the physician, parent, and child.

    • The parent or guardian must bring all of the prescribed medications to school in the prescription-labeled container.

    • All over the counter medications MUST be brought in an unopened container. Students may not self carry. 

    • All medications will be stored in the health office with the exception of medications designated in Part III, as prescribed by the physician.

    • Parent/guardian may pick up medication at the close of the school year. Medication remaining after the last day of school will be properly discarded.

    Other Physician order documents are acceptable; however, the orders must include all of the information contained on the TVUSD medication form and the parent/guardian will need to sign the TVUSD form and attach it before turning it into the health office.
