Notice of Parent’s Right to Opt Their Student Out of Taking the California Healthy Kids Survey (Grades 7,9,11) – Opt Out Forms Due by January 31, 2017

Students in grades 7, 9, and 11 are being asked to be a part the California Healthy Kids Survey (CHKS) sponsored by the California Department of Education. This is a very important survey that will help promote better health and well being among our youth, improve the school learning environment and combat problems such as drug abuse and violence. Your child does not have to take the survey.  If you do not want your child to participate in the survey, you must notify the school by submitting the Opt Out form that is attached to this email by January 31, 2017.   If the school does not receive an Opt Out form, your child will take part in the survey during the survey window of February 13-February 23, 2017.

Survey Content. The survey gathers information on developmental supports provided to youth; school connectedness and barriers to learning; school safety; and health-related concerns such as physical activity and nutritional habits; alcohol, tobacco and other drug use; and risk of depression and suicide and perceived sexual orientation.

You may examine the questionnaire in the school office or at TVUSD’s webpage    

Parents will also be asked to participate in the survey and will receive information in a separate email on a later date.
