Flu Shot Clinics For TVUSD employees
Audience: Staff

Please see attached flyers for times and dates for Certificated and Classified employees. 

 

Please note that the time period for the flu clinic at the District Office on October 23, 2017 has been changed to 1:00 pm - 5:00 pm.

 

Please be advised that if you are unable to go to these clinics and you are enrolled in SISC or CVT you can go to the following locations for a $0 copay flu shot.

 

A Voucher was provided by email to all employees. The Voucher is for CE/MGMT/CONF employees enrolled in Anthem Blue Cross with SISC only***

 

CLASSIFIED

*Classified CVT members enrolled in Anthem Blue Cross can go to local retail pharmacies such as Walgreens, Rite Aid, or CVS. You must bring your CVT card with you for a $0 copay. 

 

*Classified CVT members enrolled in Kaiser can go to Kaiser. You must bring your Kaiser card with you.

 

If you have questions you can contact CVT at 800-288-9870 or Liz Perez at CVT at 559-437-2960.

 

CERTIFICATED/MANAGEMENT/CONFIDENTIAL

*Certificated and Management/Confidential SISC members who are enrolled in Anthem Blue Cross can go to a Costco pharmacy. You must bring your SISC card and the attached voucher with you for a $0 copay. If you have questions you can contact Costco pharmacy at 951-253-6039.

 

*Certificated and Management/Confidential members who are enrolled in Kaiser can go to Kaiser for a flu shot at a $0 copay. You must have your Kaiser card with you. Please be advised, if you get your Kaiser shot during the same time that you have an office visit for other reasons, you will still be charged the office visit copay for the other visit's reasons.
