Your 5th grade child is being asked to be a part of the California Healthy Kids Survey (CHKS), sponsored by the California Department of Education. This is a very important survey that will help promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence. Your child does not have to take the survey. Participation is voluntary and requires your permission.
Permission slips will come home with your child from school or you can also complete the attached permission slip and turn it into your child's school before January 31, 2017.
Students with completed permission slips will participate in the survey between February 13 and February 23. You may examine the questionnaire in the school office or at TVUSD’s webpage https://tvusd-ca.schoolloop.com/chksstudents.
Parents will also be asked to participate in the survey and will receive
information in a separate email on a later date.
