January 29, 2016Form 1095-B mailed 
California’s Valued Trust (CVT) is required by the Affordable Care Act (ACA) to provide subscribers with important tax information about the health coverage they and/or their eligible dependents had in 2015. As their health insurance administrator, CVT mailed subscribers the Internal Revenue Service (IRS) Form 1095-B on January 29, 2016. Click here to view a sample copy of the mailing. Per IRS requirements, CVT will also send the information reported on Form 1095-B directly to the IRS.

What is Form 1095-B?
Form 1095-B is an IRS document that shows an individual, and anyone that individual had enrolled as a dependent, had health coverage considered Minimum Essential Coverage (MEC) in 2015. MEC refers to a level of health benefits that the IRS requires all individuals to have in order to avoid being charged a tax penalty for not being properly insured. Individuals are directed to keep it with their other tax-related documents.
