Key Request Forms

THIS FORM MUST BE FILLED OUT ONLINE - DO NOT PRINT FORM AND FILL OUT. INCOMPLETE OR HAND WRITTEN FORMS WILL BE RETURNED.


Electronic Forms (All High Schools, Middle Schools and Elementary Schools)

 

  1. This form must be initiated by the key holder. Third party request will not be accepted.
     
  2. Completely fill out the key request form. Give as much information as possible. Incomplete forms will be returned.
     
  3. When the form is complete hit submit form. The form will be sent on for authorization.
     
  4. If your request is approved by all parties, a key will be cut and given to your key control person. You will then be sent an email informing you your key is ready for pick up.
     
  5. You may be given a date to return your key, or at the end of the school year you may be asked to return your key.


PDF Fill and Print Forms (Charter Schools, DO & Vendors)
 

  1. Completely fill out the key request form. Incomplete or hand written forms will be returned.
     
  2. Print a copy of the completed form, then sign and date.
     
  3. For Charter Schools, give completed form to your key control person for proper routing.
     
  4. For Vendors and DO personnel, give the completed form to your district department head for routing.
     
  5. If your request is approved by all parties, a key will be cut and given to your key control person.
     
  6. You may be given a date to return your key, or at the end of the school year you may be asked to return your key.

 

MASTER KEYS ARE RESTRICTED TO AUTHORIZED SITE ADMINISTRATORS, CAMPUS SUPERVISORS, AND CUSTODIAL PERSONNEL ONLY.


All keys are the property of Temecula Unified School District and are to be returned upon request.

 

For lost or stolen keys: A statement of facts must be filled out before your key is replaced.
