TVUSD students in grades 5,7,9 and 11 are annually asked to participate in the California Healthy Kids Survey (CHKS), sponsored by the California Department of Education. The survey will be given in grades 5, 7, 9 and 11.This is a very important survey that will help promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence. The survey will be administered from February 13 through February 23, 2017. Parent permission for grade 5 and opt out forms for grades 7,9 and 11 must be received in your child's school office by January 31, 2017.
TVUSD parents of students in grades, 5, 7,9 and 11 received the permission letters regarding survey participation options through the District's Infinite Campus email system. Grade 5 parents were also sent hard copy letters through their students.
Student participation is voluntary. For students in grade 5, participation is voluntary and requires completion of the elementary parent permission letter listed below. For students in grades 7, 9 and 11, parents must notify the school by completing the CHKS Survey Withdrawal Form contained in the secondary permission letter listed below.
